Since CMS rolled out the Hierarchical Condition Category (HCC) model in 2004, accurate capture of chronic and high-impact conditions has been essential for reflecting true patient complexity. When HCCs are missed, under-documented, or not recaptured annually, practices don’t just lose data integrity, they leave revenue on the table and increase audit exposure.
Understanding how HCCs flow from documentation → coding → RAF → payment is critical for anyone involved in coding, compliance, or provider education.
